In the past 51 years, the organization has reached over 150,000 youth and countless family members by providing a variety of services aimed at fulfilling our mission of helping youth develop into responsible and productive citizens.

Services range from prevention to day treatment to residential programming. Each year around 5,000 youth who have been involved with or are at risk of entering the justice system are provided educational and behavioral services through our evidence-based, AMIkids Personal Growth Model or service coordination to meet their individualized needs and goals.

Position Summary

The role of the Clinical Case Manager is to oversee the service planning and transitional process for youth entering and exiting the program. Facilitate the case management process which includes leading the Treatment Team, develop Treatment Plans, evaluate youth needs and ensure the timely delivery of appropriate program services to meet these needs within the established guidelines of the Personal Growth Model.

Essential Job Duties

  • Oversee the intake needs assessment on youth as they enter the program, and oversee re-assessments within established contractual guidelines, Ensure all youths have received academic, vocational, and behavioral assessments within of the required timeframe of enrollment and that this data is used in development of the individual service/care plan,
  • Deliver Evidence Based Treatment models adopted by the Program, adhere to fidelity requirements of evidence based treatment models per contract,
  • Facilitate youth progress meetings, with members of the treatment team monthly, or more frequently if needed,
  • Conduct home visits with youth's families in accordance with program and state requirements, or as needed; helping impacted parties to understand the effect of group living and family separation and the transition back into the family or other residential care,
  • Schedule transitional meetings prior to release date and assists in the development of a Transition Plan prior to the youth's termination from the program, adhering to deadlines within contractual guidelines,
  • Complete administrative tasks related to the role:
    • Disbursement of comprehensive, monthly reports to the parent/guardian, judge and DJJ Probation Officer, or other applicable individuals, within required timeframe from date of the MDT meeting,
    • Ensure advisors are completing weekly behavioral up-dates and weekly goal progressions,
    • Conduct monthly audits to ensure all pertinent information is recorded,
    • Maintain case management and service notes,
    • Conduct and maintain youth exit interview information,
    • Schedule off-site appointments and transportation for youth,
  • Attend and represent the program at court hearings, meetings and youth legal proceedings,
  • Assist in establishing desirable youth conduct and discipline; prevents and intervenes in youth altercations and incidents,
  • Lead and/or participate in positive reinforcement, interdisciplinary and/or care review meetings,
  • Serve as a liaison between internal and external partners,
  • Attend required training/professional development events and maintain appropriate certification(s) and license(s),
  • Assist with special projects and other duties as assigned.

Minimum Education, Training and Experience

  • Bachelor's Degree in Psychology, Social Work, Counseling or related human services field which includes major course work in the study of human behavior and development, counseling and interviewing techniques and individual, group or family therapy,
  • Master's Degree in Psychology, Social Work, or other counseling related field preferred, license eligible is highly desirable,
  • Two (2) years' experience working with serious emotional disturbance or substance abuse problems preferred,
  • Experience working with families in the home environment, providing counseling and training is highly desirable.

Qualifications

Education
Masters of Social Work (preferred)
Bachelors of Psychology (required)

Experience
working with families in the home environment, providing counseling and training (preferred)
2 years: working with serious emotional disturbance or substance abuse problems (preferred)
